What Actually Happens to Your Body While You Sleep

Sleep is one of the most important parts of staying healthy, and one of the easiest to overlook. Most of us think of it as time when nothing much is happening. In reality, some of the most valuable work your body does takes place while you are asleep, and understanding it explains something many people notice but struggle to account for: why a poor night can leave your body feeling worse the following day.

Tissues repair, muscles rebuild, and the nervous system settles after the demands of the day. None of it is visible, and most of it goes unnoticed until it is missing.

Why Sleep Matters for Your Body

Sleep is not a pause in your body's work. It is the period when much of the repair and maintenance takes place.

During the night, your body:

  • Repairs muscle and other soft tissue

  • Regulates hormones involved in growth and recovery

  • Allows the nervous system to return to a calmer state

  • Consolidates learning, including new movement patterns

When sleep is short or broken, some of this work is left unfinished. You then begin the next day part way through a process that should have been completed overnight.

This is why poor sleep often shows up physically before it shows up anywhere else. Stiffness first thing, less tolerance for activity, and aches that feel louder than usual are all common after a bad night.

What Happens While You Sleep

Sleep is not one continuous state. It moves through cycles, and different stages do different jobs.

Deep Sleep

Deep sleep is concentrated in the first half of the night. Heart rate and blood pressure fall, breathing settles, and the body shifts into its most restorative state.

This is when most physical repair takes place. The small amounts of stress created by ordinary activity, whether that is a long walk, an afternoon of gardening or an exercise session, are addressed during these hours.

Dreaming Sleep

Dreaming sleep builds through the second half of the night and is longest shortly before you wake.

It plays an important role in consolidating memory, including motor learning, which is how a movement you have practised becomes one you can perform without thinking about it.

This matters during rehabilitation. When someone works through a set of exercises and then repeats them at home, part of that learning is secured overnight.

Because the two stages sit at opposite ends of the night, losing sleep at different times has different effects. A late night tends to reduce dreaming sleep. A broken first half of the night tends to reduce deep sleep.

Sleep and Your Muscles, Joints and Spine

Different tissues behave differently overnight.

Muscle is broken down and rebuilt continuously, and the rebuilding is concentrated during sleep. This is why a demanding session followed by a poor night can leave you feeling you have had the cost of the activity without the benefit.

Joints rely on movement rather than rest. Joint cartilage has no direct blood supply and is nourished by fluid moving in and out as you move. A long period in one position can therefore leave joints feeling stiffer rather than fresher.

Spinal discs lose a small amount of fluid across a day spent upright and take it back on while you are lying down. Most people are very slightly taller first thing in the morning as a result.

This also explains why the lower back often feels stiffest in the first half hour after getting up, and why it is sensible to avoid heavy lifting immediately after waking. A little gentle movement first is worthwhile.

Sleep and Pain

Pain is not a simple reflection of tissue damage. It is produced by the nervous system based on a range of information, including how well rested you are.

Research consistently shows that after a period of restricted or broken sleep, people report more pain from the same stimulus. The threshold moves.

The relationship works in both directions:

  • Pain can make it harder to sleep

  • Poor sleep can make pain feel more intense

This can become a cycle, and it is worth recognising. People often assume a long-standing problem is getting worse when what has actually changed is their sleep.

Addressing either side of the cycle can help. That may mean treating the source of the discomfort, or working on sleep itself. Often it means both.

Does Sleep Change as We Get Older?

Sleep does change with age. From midlife onwards, many people experience:

  • Less deep sleep

  • More frequent waking during the night

  • Earlier bedtimes and earlier waking

  • More disruption from discomfort, medication or needing the bathroom

What does not change nearly as much is how much sleep the body needs.

The idea that we need less sleep as we get older is a common one, but the evidence points to something different. The need stays broadly similar, while the ability to obtain it easily declines.

This distinction is useful. If you believe you need less sleep, a poor night is easy to accept. If you understand that the need remains and simply requires more protection, it becomes something you can act on.

Daytime activity, natural morning light, managing joint discomfort and keeping a regular rising time all support better sleep. None of them are complicated.

Recovery is Part of Staying Active

Exercise, walking, gardening and rehabilitation exercises all place demands on the body. Sleep is when the body responds to them.

If progress has stalled despite consistent effort, the limiting factor is not always the amount of work being done. Sometimes it is the recovery around it.

In clinic, the patients who make the fastest progress are rarely the ones working hardest. They are usually the ones recovering best between sessions.

We see this most clearly in people who have plateaued. The exercises are appropriate, the effort is there, and nothing is shifting. More often than expected, the conversation that helps is not about the programme at all. It is also worth saying that people who sleep badly often feel they are failing at something others manage easily. Sleep is a physiological process with genuine age-related changes behind it, not a test of character.
